Your health preferences are formally recorded in your digital Advance Care Directive, which can be securely linked to an accessible QR Emergency Access Card through Evaheld’s advanced care planning software.
This allows authorised healthcare professionals to quickly access your directive and critical health information when urgent decisions are required, helping ensure your voice is heard even in time-sensitive situations.
You can upload the statutory Medical Directive for your state or territory, or complete the Evaheld Advance Care Directive, bringing together your medical, legal and personal care preferences in one secure location.
